We are excited to announce the first Coos Bay School District Student Flu Clinic In Memory of Blake Crane. Free Flu shots are available for all students, provided by the Coos Bay School District in coordination with the Waterfall Community health center and Families Fighting Flu.
Vaccinations will be administered at your child's school in the Waterfall Community Health Centers Mobile Health Bus to maintain social distancing and school COVID policies.
Flu Vaccines area available to all Coos Bay School District students, hybrid or CDL; Parents may accompany students. Please call your school nurse and make an appointment if your student is not learning on campus at this time, and/or you would like to accompany your student.
Complete the attached consent form (Save it first to make it fillable and sign it digitally) and return to your child's school or via e-mail to [email protected] as soon as possible. If your child is attending in-person learning, printed consent forms are being sent home Monday, October 12. The Vaccine Information Sheet is attached below.
